What should you know about leg swelling after normal delivery?

Leg swelling after a vaginal delivery can result from fluid changes and reduced movement, but sudden, one-sided, painful, or worsening swelling needs prompt assessment. Swelling can affect both feet and ankles, particularly after a long labour, intravenous fluids, or time spent standing.

After birth, the body redistributes extra fluid. Hormonal changes, intravenous fluids, heat, and reduced walking can leave fluid around the ankles and feet. Swelling should gradually improve rather than steadily intensify. A clot in a deep leg vein is a different concern and often causes swelling, warmth, tenderness, or colour change on one side. The route of birth does not remove the need to check warning signs.

Rest with your legs supported, change position regularly, and take gentle walks if you feel able. Avoid tight socks or clothing around the swollen area, drink to thirst, and do not start a diuretic or compression garment without advice. Contact your maternity team if the swelling is not easing or makes walking difficult. Seek urgent help for one-sided swelling with calf pain or warmth, chest pain, breathlessness, coughing blood, faintness, or a fast heartbeat. Contact your maternity team promptly for a severe headache, vision change, upper abdominal pain, or sudden swelling of the face and hands, because these can signal a blood-pressure problem after birth.
