Leg swelling after knee surgery: what to know

Tissue irritation, reduced movement, and fluid settling in the lower leg can make the knee, calf, ankle, or foot look puffy after an operation. It may be more noticeable when the leg is down and may ease with the plan given by your surgical team. Swelling on one side with calf tenderness, warmth, redness, or a feeling of tightness is different and should be assessed promptly. A leg symptom does not by itself identify the cause.

Follow the surgeon’s instructions for walking, exercises, wound care, and prescribed medicines. Rest with the leg supported when advised, move the ankle as permitted, and avoid sitting with the knee bent for long periods. Do not massage a newly swollen or painful calf, stop prescribed clot-prevention medicine, or add compression without checking with the surgical team. Compare both legs and note when the swelling changes.

Contact your surgical team promptly for increasing swelling, worsening pain, spreading redness, wound drainage, fever, or a calf that becomes tender or warmer than the other side. Seek emergency care for sudden breathlessness, chest pain, coughing blood, faintness, or severe weakness. These symptoms can signal a serious complication and should not wait for a routine appointment.

Ask whether the pattern and location of swelling fit your recovery, which exercises and elevation position are suitable, and when the wound should be reviewed. Ask what symptoms should trigger same-day assessment and whether your medicines or existing circulation problems alter the plan.
