What the cramp may mean

Pregnancy changes posture, circulation and the workload placed on the calves and feet. A cramp may happen after activity, while resting, or when turning in bed. Dehydration, muscle strain and some mineral or vitamin problems can also contribute. Repeated cramps do not by themselves identify one condition. If they are frequent, severe or affecting sleep, a clinician can review your symptoms, medicines and pregnancy history; tests may be considered if another cause is suspected.

What you can try

When a cramp starts, straighten the leg and gently pull the toes towards your shin. Massage the tight muscle and stand or walk carefully if you can do so safely. A warm shower or covered heat pack may relax the area. Regular gentle calf stretches, comfortable footwear, changing position, and drinking fluids through the day may help prevent episodes. Do not start magnesium, calcium or other supplements without advice from your maternity clinician.

When to seek care

Contact your maternity team promptly if one leg is swollen, warm, red or persistently painful, especially if the change is new or worsening. Seek emergency help for sudden breathlessness, chest pain, coughing blood, faintness or severe weakness. Arrange a routine review when cramps keep returning, leave lasting weakness, or are accompanied by numbness, difficulty walking or reduced urine.
