What should you know about left peroneal tendonitis?

Short answer

Left peroneal tendonitis is irritation or degeneration of tendons on the outer ankle, often causing pain with walking, running or ankle movement.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Location
Behind and below the outer ankle
Common aggravators
Running, hills and uneven ground
Useful focus
Progressive ankle strength and balance

What it means

The peroneal tendons run behind the outer ankle bone and help support the foot during movement. Repeated loading, a change in training, uneven ground or an ankle sprain can aggravate them. Pain may be felt behind or below the outer ankle, with tenderness, swelling, stiffness or a snapping sensation. Weakness that makes the ankle give way can suggest a more substantial tendon problem.

A clinician compares both ankles, checks strength and balance, and may request imaging when the diagnosis is uncertain or a tear is suspected. Muscle soreness after exercise can overlap with other causes, but pain in one focused tendon area that persists or returns deserves a proper assessment.

What to do next

Reduce running, jumping, hills and uneven-surface activity while symptoms settle. Wear supportive footwear, avoid forcing painful ankle positions, and use a cold pack through fabric if it eases discomfort. Gradually resume loading only when everyday walking is comfortable and symptoms are not worsening.

A rehabilitation plan may include mobility work, balance practice and progressive calf and ankle strengthening. [Physiotherapy](/treatments/physiotherapy-and-rehabilitation/physiotherapy/) and [Strength Training Rehabilitation](/treatments/physiotherapy-and-rehabilitation/strength-training-rehabilitation/) describe ways to rebuild controlled movement. If symptoms remain despite guided care, a clinician may discuss options such as [PRP Injection](/treatments/non-surgical-orthopedics/prp-injection/) or [Shockwave Therapy](/treatments/physiotherapy-and-rehabilitation/shockwave-therapy/) based on the diagnosis.

When to seek care

Arrange prompt assessment after a sudden ankle injury with a pop, rapid swelling, inability to bear weight, repeated giving way or a new snapping tendon. Seek urgent care if the foot becomes numb, cold, pale or severely swollen, or if there is an open wound. Persistent outer-ankle pain that limits walking should be assessed rather than repeatedly pushed through.

Questions for your doctor

Ask which tendon is involved, whether there is a tear or ankle instability, what activity level is safe, and how quickly loading should progress. Ask whether footwear, an ankle brace, physiotherapy or imaging would help, and what finding would change the treatment plan.

Questions people ask

Can left peroneal tendonitis heal without surgery?

Many cases are managed with activity modification, supportive care and progressive rehabilitation, but a suspected tear or persistent weakness needs specialist review.

Should you keep exercising with peroneal tendonitis?

Choose activities that do not increase pain during or after exercise, and pause impact loading until walking and basic ankle control are comfortable.
