Leaking pee during pregnancy: what to know

Short answer
Leaking pee during pregnancy can result from pressure on the bladder and changes in pelvic floor support, but new or troublesome leakage deserves assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common triggers
- Coughing, sneezing, laughing, lifting and exercise
- Record
- Urgency, drinks, toilet visits and leakage in a short diary
What it may mean
Coughing, laughing, sneezing, lifting or exercise can increase pressure on the bladder and cause stress leakage. A sudden strong urge followed by a leak is urgency leakage. Pregnancy can also make it harder to empty the bladder comfortably. Burning, cloudy or strong-smelling urine, lower tummy pain, fever or feeling unwell can occur with a urinary tract infection, which needs prompt advice. Leakage itself does not identify one cause, so the pattern and accompanying symptoms matter.
What you can do
Keep a brief bladder diary: record drinks, toilet visits, urgency, activity and leakage. Do not severely restrict fluids, because concentrated urine can irritate the bladder. Try regular, relaxed toilet visits and avoid rushing. A pelvic health clinician or maternity team can check pelvic floor technique and tailor exercises; repeatedly tightening the wrong muscles can increase discomfort. Mention constipation, coughing and any previous pelvic floor symptoms at an appointment. Read more about [Urinary Incontinence](/symptoms/urinary-incontinence/).
When to seek care
Contact your maternity team or clinician promptly for burning urine, fever, chills, back or side pain, blood in the urine, vomiting, or difficulty passing urine. Seek urgent help if you feel very unwell, have severe pain, or notice a sudden gush of fluid that could be unrelated to urine. Arrange a routine review when leakage continues, limits activity, disrupts sleep or starts after a new medicine.
Questions for your doctor
Could this pattern fit stress leakage, urgency leakage or an infection? Should my urine be tested? Can you check my pelvic floor technique and suggest pregnancy-safe exercises? What symptoms should make me contact the maternity team immediately?

Questions people ask
Is leaking pee in pregnancy always a urine infection?
No. Pressure on the bladder and pelvic floor changes can cause leakage without infection, but burning, fever, pain or feeling unwell should prompt a urine assessment.
Can pelvic floor exercises help during pregnancy?
They may help some types of leakage when taught and performed correctly. Ask a maternity or pelvic health clinician to check your technique.
