What should you know about an LDL-C calculated blood test?

How calculated LDL-C works

The laboratory uses measured parts of the lipid profile, commonly total cholesterol, HDL cholesterol and triglycerides, to estimate LDL-C. It is therefore different from a directly measured LDL result. The estimate may be less reliable when triglycerides are markedly changed or the sample conditions are unusual. Your report may also show a non-HDL cholesterol value, which gives another view of cholesterol carried by potentially harmful particles.

The result is interpreted with blood pressure, diabetes, smoking, kidney health, family history and previous vascular disease. A calculated value cannot by itself diagnose blocked arteries or explain symptoms.

Using the result well

Ask which calculation was used and whether the triglyceride level makes the estimate dependable. Bring previous results and a list of medicines, supplements and recent illnesses. Follow the agreed plan for food, activity and prescribed treatment; do not start, stop or change medicine from the calculated number alone. If the result conflicts with your clinical picture, the clinician may repeat the profile or request a direct measurement.

When to seek urgent care

The calculated result itself is not an emergency. Seek immediate help for chest pressure, severe breathlessness, fainting, sudden weakness, facial drooping or trouble speaking. Those symptoms need urgent evaluation whether or not the lipid report has been completed.

Questions worth asking

Ask whether your sample required fasting, which lipid values fed into the estimate, and whether your triglycerides affect confidence in it.
