Late period and heavy bleeding: what to know

When ovulation happens later than usual, the uterine lining may build up and the next bleed can feel heavier or last longer. Hormonal transitions such as perimenopause can make periods less predictable. Polycystic ovary syndrome may also cause widely spaced periods followed by heavier bleeding. Pregnancy should be considered when a period is late, because bleeding in pregnancy has different causes and may need urgent assessment.

Note when the bleeding began, how often you change pads or tampons, whether there are clots, and whether you have pain, dizziness, fever, or unusual discharge. These details help a clinician judge the pattern.

Take a pregnancy test if pregnancy is possible, unless a clinician has already advised you otherwise. Rest, drink fluids, and use period products that let you track the amount of bleeding. Do not take medicines containing aspirin unless a healthcare professional has told you to use them, because they can increase bleeding. Arrange a medical review for a new, recurrent, or disruptive pattern; assessment may include a pregnancy check, blood tests, and an examination.

Seek urgent help if bleeding is soaking through protection rapidly, you feel faint or short of breath, you have severe or one-sided abdominal pain, or you may be pregnant. Go promptly as well for heavy bleeding with marked weakness, a racing heartbeat, or shoulder-tip pain. These features need assessment for blood loss or a pregnancy complication rather than watchful waiting.
