What should you know about late period after miscarriage?

After a miscarriage, pregnancy hormones gradually fall and the menstrual cycle may take time to settle. Ovulation can happen before the first period, so a new pregnancy is possible even when bleeding has not returned. Timing also varies with the stage of pregnancy, treatment received, breastfeeding, stress and the return of usual hormone patterns.

A missed period commonly presents as delayed or absent menstrual bleeding, but it does not identify one cause by itself. Perimenopause and Polycystic Ovary Syndrome can also be associated with irregular cycles, although the context of a recent miscarriage matters.

Note the miscarriage date, how bleeding changed, when it stopped, and any pain, fever or unusual discharge. If pregnancy is possible, use a home pregnancy test according to its instructions and tell the clinician about the result. Avoid vaginal sex or inserting anything if your clinician advised waiting, and follow any planned review or scan.

Arrange a review if the period does not return when your clinician expected, if a test is positive or unclear, or if you remain worried. Seek urgent medical help for heavy bleeding, severe or one-sided pelvic pain, shoulder-tip pain, fainting, severe dizziness, fever, chills, or foul-smelling discharge. These symptoms can signal bleeding, infection or an ectopic pregnancy and should not be explained away as a late cycle. Contact your maternity or gynaecology team promptly if pregnancy symptoms return or pain is increasing.

Ask when your period should reasonably return in your circumstances, whether another pregnancy is possible, and whether you need a pregnancy test, examination or scan. Mention the miscarriage treatment, your blood group if known, contraception, breastfeeding and every symptom that has changed.
