What should you know about laser treatment for old burn scars?

A healed burn scar can remain red, darker than nearby skin, raised, firm, indented, itchy, or tight. Different laser approaches target different features: some reduce visible redness, while fractional resurfacing creates controlled microscopic treatment zones that encourage remodelling in uneven or stiff scar tissue. A scar that crosses a joint or limits movement needs assessment for function as well as appearance.

Treatment is usually planned as a series rather than a single visit, with settings adjusted for scar maturity, thickness, body site, and your natural pigmentation. Improvement is gradual. Temporary swelling, redness, tenderness, crusting, or colour change can occur, and darker skin tones may need especially cautious settings and aftercare.

Bring the burn history if you have it, including operations, grafting, infection, and previous scar treatments. Tell the clinician about medicines, keloid or raised-scar history, cold sores, eczema, recent tanning, and any change in the scar. Sun protection helps reduce contrast between treated and untreated skin. Follow wound-care instructions closely and do not pick peeling skin.

A dermatologist may discuss fractional laser treatment, scar revision surgery, or a skin graft when a scar is very tight, bulky, or function-limiting. These options address different problems and are not interchangeable.

Arrange prompt medical review if an old scar develops a new ulcer, bleeding, increasing pain, drainage, foul smell, rapid enlargement, or a marked change in colour or surface. Seek urgent care for spreading redness with fever, severe swelling, or difficulty moving a nearby joint. These signs need assessment before cosmetic laser treatment.
