Laryngitis and hoarseness: what should you know?

Hoarseness may sound rough, strained, weak or breathy, and the pitch can change. Laryngitis may follow a cold, heavy voice use or irritation from reflux and smoke. Persistent hoarseness has other possible causes, including vocal-fold problems, thyroid conditions or disease affecting the throat or lungs; a lasting voice change deserves assessment rather than self-diagnosis.

Use your voice gently and avoid shouting, singing and whispering while it is recovering. Drink water, reduce exposure to smoke and take breaks from speaking. Treating nasal drainage or reflux may help when those irritants are present, but use medicines only as advised. Keep track of when the hoarseness started, what worsens it and whether swallowing or breathing has changed.

Seek urgent help if hoarseness comes with breathing difficulty, noisy breathing, inability to swallow, drooling, coughing blood or sudden swelling. Arrange a medical review for a persistent or recurrent voice change, especially with a neck lump, unexplained weight loss, pain when swallowing, ear pain or a long smoking history. An examination may include looking at the throat and deciding whether the vocal folds need specialist review.

Ask what may be irritating your vocal folds, whether reflux or a respiratory condition is involved, and when further examination is appropriate. Describe your voice demands, smoking exposure, medicines and any throat, chest or neck symptoms.
