Large red spots on skin: what to know

A raised spot containing pus may fit acne or folliculitis, especially around hairs or after shaving. Recurrent spots along the jaw can occur with hormonal acne. Redness and flushing across the cheeks or nose may occur with rosacea, while a rash around the mouth has other possible causes. A spot that is hot, increasingly tender, rapidly expanding, or accompanied by drainage needs a different assessment from a stable blemish. Note whether the area itches, feels painful, forms scale, follows a new product, or affects one or both sides.

Avoid squeezing, scratching, scrubbing, or repeatedly touching the area, since friction can worsen inflammation and leave discoloration. Wash gently with lukewarm water and a mild cleanser, then pause newly introduced cosmetics or active products until the skin settles. Use a clean, cool compress for short periods if the area feels warm or irritated. Keep pillowcases, razors, and phone screens clean, and do not share towels. A clinician can distinguish acne from folliculitis, rosacea, or perioral dermatitis and choose treatment accordingly.

Arrange prompt medical review for redness that spreads quickly, severe pain, marked warmth, fever, facial swelling, eye involvement, or red streaking. Seek urgent help for trouble breathing, swelling of the lips or tongue, faintness, or a rapidly developing widespread reaction. Book a routine dermatology appointment when spots keep returning, leave scars, persist despite gentle care, or are difficult to identify.

Ask whether the pattern fits acne, folliculitis, rosacea, or another rash; which ingredients or habits may be aggravating it; and whether a swab, examination, or treatment trial is appropriate. Bring a list of new products, medicines, shaving practices, and the dates when the spots appeared.
