What should you know about a large lump on the neck?

A large lump on the neck needs a clinical assessment because its position, texture, growth and associated symptoms guide the next steps. Many neck lumps have causes other than cancer, but a persistent or enlarging mass should not be self-diagnosed.

A neck lump may be an enlarged lymph node responding to infection, a thyroid swelling, a cyst, or another benign growth. A lump can also be a presentation of head and neck cancer, lymphoma, thyroid cancer, or cancer of unknown primary. Location matters: a clinician may examine the mouth, throat, thyroid, skin and nearby lymph-node areas. Firmness, mobility, tenderness and whether the mass is changing help determine which tests are useful.

Arrange an appointment with a doctor, especially if the lump remains, grows, feels firm or is not linked to a short-lived illness. Record when you first noticed it, any change in size, pain, fever, night sweating, swallowing difficulty, voice change or unexplained weight change. Do not squeeze or repeatedly press the area. Examination may be followed by imaging or a tissue sample, depending on the findings. If a thyroid cause is suspected, ask how thyroid testing and specialist review fit into the plan.

Seek urgent medical help if the swelling causes trouble breathing, severe difficulty swallowing, rapidly worsening pain, marked redness, or sudden enlargement. Contact a clinician promptly for a lump that persists, keeps growing, is hard or fixed, or comes with ongoing hoarseness, unexplained fever or drenching night sweats. These features do not identify the cause by themselves, but they make timely assessment more important.

Ask: Where is the lump most likely arising from? Do I need an ultrasound, scan or biopsy? What symptoms should make me seek urgent help? If the lump is near the thyroid, would thyroid evaluation or thyroid removal surgery ever be considered? Which changes should I record while waiting for results?
