What should you know about a large lump at the base of the neck?

The base of the neck lies near the collarbones, thyroid gland, lower cervical lymph nodes and the upper chest. A lump here may be related to infection, a thyroid change, an enlarged lymph node or another benign or serious condition. A neck lump can be a presentation of lymphoma, thyroid cancer or head and neck cancer, but location and size cannot establish a diagnosis. If no source is found, clinicians may investigate cancer of unknown primary.

A clinician may check the neck, mouth and throat, then arrange ultrasound, blood tests, a needle sample or other imaging. The next test depends on the examination and how the lump behaves over time.

Record when you first noticed the lump, whether it is changing, and any fever, night sweats, voice change, cough, swallowing difficulty or recent infection. Avoid pressing or repeatedly checking it. Take previous reports and a medication list to the appointment. Ask how results will be communicated and keep follow-up arranged if the first test does not explain the lump.

Get emergency help for breathing difficulty, inability to swallow saliva, sudden rapidly spreading swelling or fainting. Seek prompt medical advice if the lump enlarges, becomes painful or red, or occurs with persistent hoarseness, swallowing problems, fever, unexplained weight loss or marked night sweats.

Ask which structures are near the lump, whether thyroid or lymph-node testing is needed, when imaging or sample results should arrive, and what symptoms should lead you to seek earlier care.
