What should you know about a large hard lump on the neck?

The neck contains lymph nodes, the thyroid gland, salivary glands, muscles and blood vessels. A lump may follow an infection, reflect an enlarged lymph node, or arise from the thyroid or another tissue. A hard or fixed lump, a lump that persists or grows, or one accompanied by voice change, swallowing difficulty or unexplained weight loss deserves timely review. A neck lump can be a presentation of head and neck cancer, lymphoma or thyroid cancer, but its texture alone cannot identify the cause.

Assessment may include an examination of the mouth and throat, a neck ultrasound, blood tests and a needle sample. Further imaging or specialist review may be recommended according to the findings.

Arrange an appointment and note when the lump appeared, whether it has changed, and any recent infection, dental problem, smoking or voice symptoms. Do not repeatedly squeeze or massage it. Bring medication details and previous imaging. Keep the follow-up appointment even if the lump is not painful, because painless lumps can still need explanation.

Seek emergency help if the lump causes difficulty breathing, inability to swallow saliva, rapidly increasing swelling or severe weakness. Contact a clinician promptly for new bleeding, persistent hoarseness, worsening pain, fever or a lump that continues to enlarge.

Ask which structure the lump appears to involve, whether ultrasound or a needle sample is needed, how soon results will be available, and what changes should prompt earlier review.
