What should you know about large cyst on scalp?

A pilar cyst, also called a trichilemmal cyst, is a smooth, firm lump that develops around a hair follicle. It is usually covered by normal-looking skin and may move slightly under the fingers. Several cysts can occur on the scalp, and friction from combing, helmets, or hair accessories can make one sore.

Pain, warmth, redness, tenderness, or drainage can indicate inflammation or infection rather than a quiet cyst. A clinician can assess whether the lump is superficial and cyst-like or whether another type of skin lump needs consideration.

Avoid squeezing, puncturing, or repeatedly pressing the lump. Protect it from combs and tight headwear, keep any opening clean and covered, and arrange an examination if it is enlarging or troublesome. Do not apply harsh chemicals or attempt to remove the cyst sac yourself.

If removal is appropriate, cyst removal usually involves numbing the area and taking out the cyst and its wall. Removing the wall helps reduce the chance of the lump returning, although healing time and the resulting scar vary with the cyst and its position.

Seek prompt care for rapid enlargement, severe pain, spreading redness, pus, fever, or a foul smell. Get urgent help if swelling is associated with severe headache, confusion, weakness, or an injury. A hard, fixed, bleeding, or persistently changing scalp lump needs direct assessment even when it is not painful.

Ask whether the lump has features of a pilar cyst, whether it should be removed now or observed, what type of anaesthetic and wound care are involved, and how hair washing and combing should be managed afterward.
