The cough can sound rough or bark-like, with hoarseness and a harsh noise when breathing in. It may start after cold symptoms and often becomes more obvious overnight. Croup affects the upper airway, while illnesses such as bronchiolitis or asthma affect breathing in different ways. A cough can also occur with acute bronchitis or a common cold, so the child’s breathing pattern and condition matter more than the label.

A mild episode may involve a cough only when upset or active. Noisy breathing while the child is settled, visible effort with each breath, or difficulty drinking suggests more significant narrowing and needs medical attention.

Stay close, speak softly, and keep the child sitting upright. Offer cool fluids in small amounts if swallowing is comfortable. Keep the room free from cigarette smoke and avoid forcing food. Do not give antibiotics or cough remedies unless prescribed for this child. If the symptoms are new, contact a healthcare professional for advice about observation and treatment.

Call emergency services if the child is gasping, has blue or grey colouring, cannot swallow saliva, becomes hard to wake, or has pauses in breathing. Get urgent assessment if the skin sucks in around the ribs or neck, the noisy breath continues while resting, or the child cannot drink. Do not drive alone if breathing is rapidly deteriorating; keep the child upright while help is arranged.
