Where swelling can occur
Swelling directly over the kneecap can fit knee bursitis, especially after kneeling or pressure. Pain along the tendon below the kneecap may occur with Patellar Tendinitis, while pain around the kneecap can occur with Chondromalacia Patellae. Fluid within the joint may make the whole knee look puffy and can accompany osteoarthritis, gout, a meniscus tear, or an ACL tear. A Baker's Cyst creates fullness behind the knee.
Notice whether the area feels soft or firm, whether the skin is hot or red, and whether the swelling moves with the skin or seems deep. These observations help a clinician focus the examination, but they cannot confirm a diagnosis alone.
How to protect the area
Avoid the pressure or repeated motion that brought on symptoms, such as kneeling, jumping, or deep bending. Support the leg when resting and try a cloth-wrapped cold pack for brief periods if comfortable. Keep gentle movement within an easy range, but stop if pain increases. Arrange an assessment when swelling persists, expands, recurs, or changes the way you walk. Imaging or joint aspiration may be considered when swelling seems to come from inside the joint.
Signs that need prompt care
Seek urgent assessment when swelling is hot, red, rapidly worsening, or accompanied by fever, severe pain, or feeling unwell. Get immediate help if the knee is locked, you cannot bear weight, or calf swelling occurs with chest pain or breathlessness. A local lump still needs review if it becomes increasingly tender or limits movement.
