Knee swelling after exercise can result from joint irritation, an overworked tissue, or an injury that needs assessment. Notice when the swelling starts, whether the knee feels hot, and whether you can fully bend and straighten it.

What the swelling may mean

Swelling may be fluid inside the joint or inflammation in tissues around it. Repeated loading can aggravate knee osteoarthritis or knee bursitis. A twisting movement with a pop, giving way, or catching can occur with an ACL tear or meniscus tear. Sudden warmth and marked tenderness can occur with gout, while fullness behind the knee has different possible causes such as a Baker's cyst. The swelling pattern does not identify one diagnosis by itself.

A clinician may compare both knees, check movement and stability, and decide whether imaging or fluid testing is useful. Keep a note of the activity, footwear, pain location, and how long the swelling lasts.

Pause the activity that brought on the swelling and choose gentle movement that does not increase pain. Rest with the leg supported, use a wrapped cold pack for short periods, and avoid placing ice directly on skin. Compression may help if it is comfortable and does not cause numbness or color change. Resume exercise gradually only when walking and everyday bending are comfortable; reduce impact and review technique if symptoms return.

When to seek care

Seek prompt medical care if the knee cannot bear weight, becomes locked, looks deformed, swells rapidly, or pain is severe after a twist or fall. Get urgent help for a hot swollen knee with fever, or for calf swelling, chest pain, or breathlessness. Arrange an assessment if swelling keeps returning, limits normal movement, or does not settle with reduced activity.
