Short answer

Knee joint pain with swelling can have several causes, so the pattern, trigger, and other symptoms guide the next step.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Track
Onset, warmth, redness, movement, and ability to bear weight
Avoid
Sport and deep bending while pain or swelling is increasing
Urgent sign
A hot swollen knee with fever or severe pain

What the swelling may mean

Swelling may come from fluid inside the joint, irritation around the kneecap, a strained ligament, or a blow to the knee. Sudden severe pain with a hot, red joint needs prompt medical assessment because infection or crystal inflammation such as gout can affect joint movement. Fever, chills, or feeling markedly unwell adds concern. Aches in several joints can occur with illnesses such as influenza, dengue fever, brucellosis, or Long COVID. Fibromyalgia can cause widespread pain, although visible knee swelling points to another process that should be assessed. Weight carried by the knee can also worsen symptoms, including with obesity, while hypothyroidism may contribute to muscle and joint complaints.

What you can do now

Reduce activities that increase pain and use a cool pack wrapped in cloth for short periods. Elevate the leg when resting, and keep gentle movement within a comfortable range so the joint does not become stiff. Do not massage a hot, newly swollen knee or return to sport while walking remains difficult. Arrange a GP consultation if swelling persists, keeps returning, or follows an injury; the clinician may examine stability and movement and decide whether imaging or blood tests are useful.

When to seek care

Seek urgent care for a knee that is suddenly very swollen, hot, red, or extremely painful, especially with fever or chills. Get prompt assessment after a fall or twist if you cannot bear weight, the knee looks deformed, or it locks. Also seek help for calf swelling, chest symptoms, or rapidly worsening pain. These signs need assessment because injury, infection, or a circulation problem can require timely treatment.

Questions for your doctor

Ask what pattern of swelling fits your examination, whether you need imaging or blood tests, which activities are safe, and what changes should trigger review. Tell the clinician when the swelling began, whether the knee is hot, any injury or recent illness, and every medicine you take.

Questions people ask

Can knee swelling happen without an injury?

Yes. Inflammation, infection, crystal irritation, and some wider illnesses can cause swelling without a clear injury.

Should I keep moving a swollen knee?

Gentle movement within comfort is reasonable, but stop activities that increase pain and seek assessment if movement or weight-bearing is difficult.
