What should you know about kidney pain during sex?

Deep penetration can place pressure on the cervix, uterus, ovaries, pelvic floor muscles, or tissues behind the vagina. Pain may be felt in the lower back or flank rather than exactly where it begins. A urinary infection, bladder irritation, muscle spasm, or constipation can also make sexual activity uncomfortable.

Stop or change activity when pain begins; continuing through it can increase muscle guarding. Try slower movement, positions that limit depth, communication with your partner, and lubricant if dryness is present. Note whether pain occurs at entry, with deep penetration, after sex, or with urination, and whether it follows your cycle.

Book a confidential clinical assessment if pain returns, causes bleeding, or affects intimacy. Examination and tests may include urine testing, infection checks, a pregnancy test, or pelvic imaging. Treatment depends on the cause; some people benefit from pelvic floor therapy, medication, or specialist care.

Seek urgent help for severe sudden pain, fever, faintness, repeated vomiting, heavy bleeding, unusual discharge with illness, or pain during a possible pregnancy. Prompt review is also needed for new flank pain with burning urination or blood in the urine.

Ask whether the pain sounds superficial, deep pelvic, muscular, or urinary; whether infection testing is needed; and whether endometriosis, dryness, or a cyst should be considered. Ask when sex can safely resume if treatment is started.
