What the swelling may mean

Fluid can collect in the tissues around the ankles, feet and lower legs. Swelling related to kidney disease may affect both sides and can be more noticeable after standing, although the pattern varies. Reduced kidney filtering can alter salt and water balance; damage to the kidney filters can also allow protein to pass into urine, lowering the force that keeps fluid in the bloodstream.

Leg swelling has other possible causes, including heart failure, obesity, varicose veins, some medicines and prolonged sitting. Swollen ankles alone cannot identify the cause. A clinician may ask about urine changes, breathlessness, weight change, blood pressure and medical history, then arrange urine and blood tests.

Arrange a clinical review if swelling is new, persistent, or returning. Take a list of medicines and mention reduced urination, foamy urine, recent illness, or a known kidney condition. Do not start water tablets, stop prescribed treatment, or sharply restrict fluids without advice, because the right plan depends on kidney function and other conditions.

While waiting for advice, avoid long periods in one position and keep the legs raised when comfortable. Protect swollen skin from cracking and note whether the swelling changes during the day. Keep a record of symptoms rather than relying on a single day’s appearance.

When to seek urgent care

Seek urgent help for swelling with trouble breathing, chest pain, fainting, confusion, a marked reduction in urine, or rapidly increasing swelling. One-sided swelling with warmth, redness or significant pain also needs prompt assessment. These features can point to a serious fluid, heart, lung or blood-vessel problem and should not be judged by swelling alone.
