Kidney disease and acid reflux: what to know

Acid reflux happens when stomach contents move back into the oesophagus, causing burning behind the breastbone, a sour taste, regurgitation or symptoms after meals and when lying down. Kidney disease does not usually cause reflux directly. However, people with kidney disease may have other symptoms, medicines, dietary restrictions or fluid guidance that affect how digestive symptoms are experienced and managed.

Chest tightness, cough, abdominal pain and difficulty sleeping can have many causes. Reflux can irritate the throat and trigger cough, especially at night, while breathlessness or chest pressure may signal a heart or lung problem rather than reflux. Kidney disease may also cause swelling, changes in urine or tiredness, so these symptoms should be described separately.

Keep a brief symptom record: meals, body position, night symptoms, sour taste, cough, swelling, urine changes and medicines. Raise the head of the bed if night reflux is a problem, avoid lying down soon after eating, and discuss meal changes with your kidney clinician if you have a prescribed diet.

Ask a pharmacist or clinician before using antacids or reflux medicines. Some products contain minerals or salts that may need caution in kidney disease, and medicines can interact with other treatment. Do not stop prescribed kidney medicines because of heartburn without advice.

Get urgent help for chest pressure, severe chest tightness, fainting, sweating, blue lips, severe breathlessness, vomiting blood or black stools. Seek prompt medical advice for repeated vomiting, trouble swallowing, unexplained weight loss, new swelling or a marked reduction in urine.

A clinician should review persistent reflux, particularly when symptoms are new, worsening or disturbing sleep. Emergency symptoms should not be attributed to reflux without assessment.

Ask whether your symptoms fit reflux, whether kidney function changes the choice of treatment, and which over-the-counter products to avoid. Ask whether cough, chest symptoms or urine changes need separate testing.
