What kappa and lambda show
Plasma cells make antibodies that contain either kappa or lambda light chains. In many reactive areas, a mixture of cells is present. A tissue sample in which one light-chain pattern dominates may be described as restricted. This finding can support a clonal plasma-cell or B-cell process, but it must be interpreted with the cell appearance, the amount of tissue and other laboratory results.
Staining can be difficult when there are very few plasma cells, when tissue is damaged, or when background material obscures the pattern. A mixed pattern does not by itself exclude disease, and a restricted pattern does not name the exact condition.
What happens after the stain
The pathologist may compare kappa and lambda stains with other markers and the biopsy findings. Your clinician may combine the report with blood or urine tests, imaging and symptoms before deciding whether more assessment is needed. Ask for the exact wording: terms such as mixed, restricted, equivocal or insufficient describe different levels of certainty.
Tell the team about previous biopsies and haematology results, since comparison can help explain a new finding. Do not change medicines based on a stain result without discussing the complete report with your clinician.
When to seek care
The stain itself does not cause symptoms, but the biopsy site may need attention. Contact the care team for persistent bleeding, increasing redness, drainage, fever or worsening pain after the procedure. Seek prompt medical review for new marked fatigue, repeated infections, unexplained bruising, bone pain or changes in urination, because these symptoms need assessment rather than interpretation from the stain alone.
