Short answer

James Cyriax manual therapy is a hands-on approach that assesses painful tissues and applies targeted techniques to support movement and function.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What the approach involves

The clinician considers your history, movement pattern, strength, and the location of tenderness before choosing a technique. Treatment may include guided joint movement, soft-tissue work, stretching, or a carefully selected exercise. The aim is to address a suspected muscle, tendon, ligament, or joint contribution rather than massage every area indiscriminately. It may be discussed as part of care for conditions such as cervical spondylosis, myofascial pain syndrome, sacroiliac joint dysfunction, or whiplash.

How to prepare and participate

Tell the physiotherapist where symptoms began, what movements aggravate them, and whether you have numbness, weakness, fever, recent trauma, or a medical condition affecting healing. Wear clothing that allows the relevant area to be examined. During treatment, describe sharp, spreading, or unusual pain promptly; a technique can be adjusted or stopped. Home exercises and gradual activity are often used alongside hands-on care so gains can carry over into daily tasks.

When to seek medical assessment

Arrange prompt medical assessment for new loss of strength, worsening numbness, loss of bladder or bowel control, severe pain after an injury, or a limb that becomes cold or changes colour. A clinician should also review persistent pain, unexplained swelling, or symptoms that are steadily worsening before manual treatment continues.

Questions to ask

What tissue or movement pattern are you assessing? Which technique is planned, and what should I feel during it? What exercise or activity should follow the session? How will we judge whether the plan is helping, and what symptoms mean I should stop and contact you?
