IUD for heavy periods

A hormonal IUD releases progestogen in the uterus. This makes the womb lining thinner, so less tissue is shed during a period. Bleeding may become lighter over time, although spotting or unpredictable bleeding can happen after insertion. The device also provides contraception, which may be useful if pregnancy is not currently planned.

Heavy bleeding can be related to fibroids, adenomyosis, endometrial polyps, hormonal changes, or other conditions. Ongoing blood loss can contribute to iron deficiency anaemia, with tiredness, reduced stamina, headaches, or breathlessness. Treating the bleeding and checking its cause are separate parts of care.

Tell your clinician how often you change pads or tampons, whether you pass clots, how long bleeding lasts, and whether you bleed between periods or after sex. Review pregnancy plans, pelvic symptoms, medicines, and any previous IUD problems. A clinician may examine you or arrange tests before insertion. Keep a bleeding diary after insertion so you can discuss whether the pattern is improving.

Seek urgent care if bleeding is heavy enough to make you feel faint or weak, if you have severe pelvic pain, fever, a positive pregnancy test with pain or bleeding, or if the device has come out. Arrange review for persistent bleeding that is not settling, new bleeding after a stable pattern, or symptoms of anaemia such as breathlessness or marked fatigue.
