Itchy Lump on the Back of the Neck: What to Know

A bump in the skin may be an insect bite, follicle inflammation, eczema, or a cyst. A deeper, tender lump can be a lymph node reacting to an infection in the scalp, throat, or skin. Lymphoma, thyroid cancer, head and neck cancer, and cancer of unknown primary can also involve neck lumps, but an itchy lump alone cannot identify any of them. Size, firmness, mobility, duration, and symptoms such as fever or night sweats help a clinician decide what to do next.

Do not squeeze, puncture, or repeatedly press the lump. Keep the area clean, avoid new hair or skin products, and note when it appeared, whether it is growing, and whether the skin is red or broken. Book an appointment if it lasts beyond a short period, enlarges, feels hard or fixed, or returns after settling. Assessment may include examination of the scalp, mouth, throat, thyroid, and lymph-node areas, followed by imaging or sampling when indicated.

Seek urgent care for trouble breathing or swallowing, rapidly spreading redness, high fever, severe pain, or swelling of the face or neck. Arrange prompt review for unexplained weight loss, drenching night sweats, persistent fever, voice change, or a lump that continues to enlarge. These warning signs do not prove cancer; they indicate that an examination should not be delayed.

Ask whether the lump appears to be in the skin or beneath it, which nearby infections could explain it, and whether observation, imaging, or a biopsy is appropriate. Ask what change in size, texture, or symptoms should bring you back sooner.
