Itchy and Flaky Scalp: Causes and Care

Short answer
An itchy, flaky scalp commonly reflects dandruff or another inflammatory scalp condition, while hair loss, sores, or persistent symptoms need assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common clues
- Dandruff can cause itch with loose or greasy flakes.
- Hair change
- Broken hairs or patchy hair loss should be assessed.
- Hygiene
- Do not share hair tools or headwear if infection is possible.
What the flakes may indicate
Dandruff and seborrheic dermatitis can produce loose flakes with itch, sometimes alongside greasy scale. Scalp psoriasis may create thicker, sharply defined scale that extends beyond the hairline. A fungal infection called scalp ringworm can cause scaling, broken hairs, tender areas, or patches of hair loss and needs medical treatment.
The appearance, location, and texture of scale help distinguish these conditions, but overlapping symptoms are common. Scratching can create small wounds and crusting, making the scalp more irritated than it was initially.
Practical scalp care
Wash gently with lukewarm water and avoid picking at scale. Rinse styling products thoroughly and pause any product that burns or worsens the itch. A medicated shampoo may help some causes; use it according to its instructions and allow the active cleanser the recommended contact time before rinsing.
Do not share combs, brushes, hats, or towels if infection is possible. Arrange a review before using strong steroid or antifungal products on a child, a broken scalp, or an area with hair loss.
When to seek care
See a clinician if flakes persist despite careful washing, the scalp is painful, swollen, bleeding, or oozing, or patches of hair are breaking or falling out. Prompt advice is also appropriate for tender lumps, swollen neck glands, or a suspected infection. Seek urgent help for rapidly spreading swelling, fever, or feeling very unwell.
Questions for an appointment
Ask which pattern your scalp shows, whether a medicated shampoo is suitable, and how long to try it. Explain when the flaking began, whether it affects the face or ears, whether hair is shedding, and which shampoos or treatments have already been used.

Questions people ask
Is an itchy flaky scalp always dandruff?
No. Seborrheic dermatitis, psoriasis, and scalp ringworm can look similar but require different care.
Can I scratch scalp flakes off?
Avoid picking because it can cause small wounds, crusting, and extra irritation.
