IT Band Tendonitis in the Hip

Short answer
IT band tendon irritation near the hip may cause outer-hip tenderness and pain with loading, but similar symptoms can come from nearby tissues.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What tendon irritation involves
Tendinopathy describes a tendon that is not tolerating its current load well. Around the hip, pain may be felt over the outer bony area and may increase with stairs, running, prolonged standing, or lying on that side. It can feel stiff at first movement and more sore after a demanding session. The IT band itself is connective tissue, while nearby hip muscles and their tendons help control it. Because bursae, hip joints, and lower-back nerves can create similar symptoms, location alone cannot identify the source. An examination looks at strength, resisted movement, gait, and the effect of graded loading.
Managing the load
Avoid sudden spikes in running, jumping, or hill work and reduce side-lying pressure during a flare. Do not repeatedly stretch or massage directly into a sharp tender spot. [Physiotherapy](/treatments/physiotherapy-and-rehabilitation/physiotherapy/) can set a progressive loading plan that begins with tolerable isometric or controlled strengthening and advances as symptoms allow. [Strength Training Rehabilitation](/treatments/physiotherapy-and-rehabilitation/strength-training-rehabilitation/) may restore hip and pelvic control. [Shockwave Therapy](/treatments/physiotherapy-and-rehabilitation/shockwave-therapy/) or [PRP Injection](/treatments/non-surgical-orthopedics/prp-injection/) may be discussed for selected persistent tendon problems, after diagnosis and informed consent.
When to seek help
Arrange an assessment when pain persists despite sensible load reduction, interferes with sleep or walking, or repeatedly returns when activity resumes. Seek prompt care for a fall followed by inability to bear weight, sudden severe pain, fever, marked redness or swelling, new weakness, or numbness. A clinician can identify whether the problem is tendon-related and whether imaging is needed. Treatment should match the confirmed pain source rather than the label alone.
Questions for your doctor
Which tissue is most likely irritated? What load can I safely keep doing? How should strengthening progress? When would imaging or an additional treatment be reasonable?

Questions people ask
Is IT band tendonitis the same as muscle soreness after exercise?
Not necessarily. Exercise soreness is usually diffuse and temporary, while focal pain that returns with loading needs a different assessment.
Will stretching cure hip tendon irritation?
Stretching may not address the load problem and can aggravate a tender area; a graded strengthening plan is often more relevant.
