IT band pain after knee replacement can reflect altered movement or soft-tissue irritation, but new or worsening postoperative pain should be reviewed by the surgical team. Recovery changes how you walk, bend, and load the outside of the knee. Soreness may come from healing tissues or compensation, yet the timing and associated symptoms matter.

After surgery, swelling, reduced strength, and a changed walking pattern can place extra demand on the outer thigh and knee. Pain can also come from tissues near the kneecap, including knee bursitis or patellar tendinitis. A painful, warm, red knee with fever raises concern for infection and needs urgent clinical attention. Other knee conditions such as gout, a Baker's Cyst, or a meniscus tear may be considered depending on the examination, although they are not diagnosed from the pain location alone.

Follow the surgeon's weight-bearing and exercise instructions, and use your prescribed walking aid until your team says it is safe to stop. Break exercises into manageable sessions, elevate the leg when advised, and use cold therapy only as directed with skin protection. Do not add forceful stretches, deep massage, or extra resistance without approval. Contact the surgical team if pain is steadily increasing, prevents the rehabilitation exercises, or is different from the expected recovery pattern.

Seek urgent help for fever, increasing redness or drainage from the wound, sudden severe pain, a newly swollen calf, chest pain, or shortness of breath. Promptly report a knee that becomes unstable, cannot bend as before, or suddenly loses function. Your postoperative team can distinguish expected soreness from a problem that needs examination.

Ask whether the pain is coming from the IT band, the kneecap, the implant area, or another structure; whether your walking pattern needs correction; and which exercises are appropriate now. Ask what amount of swelling is expected at your stage and which symptoms require same-day contact.
