What should you know about ischemic bowel resection?

Short answer

Ischemic bowel resection removes damaged bowel when reduced blood flow threatens the intestine, and urgent assessment is essential.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Main purpose
Remove bowel that cannot recover from poor blood flow
Possible outcome
A join between bowel ends or a temporary or longer-term stoma
Urgency
Rapid medical assessment is needed when bowel ischaemia is suspected

What the operation means

Ischaemia means that part of the bowel is not receiving enough blood and oxygen. If the tissue cannot recover, a surgeon removes the affected segment to limit leakage, infection, or worsening injury. The remaining ends may be joined together, or the bowel may be brought to the skin as a stoma when healing conditions are not suitable for an immediate join. The exact approach depends on the location and extent of injury, your general health, and what the surgeon finds during the operation.

The cause is investigated alongside treatment. Possible contributors include a blocked blood vessel, low circulation, or pressure within the abdomen. A removed specimen may be examined to clarify the diagnosis. Ischemic bowel resection is different from planned surgery for a tumour, although a bowel condition may be discovered during assessment.

What to do before and after surgery

Because bowel ischaemia can progress quickly, follow the emergency team’s instructions about assessment, scans, blood tests, medicines, and fasting. After surgery, expect monitoring of pain, fluid balance, bowel function, the wound, and any stoma. Gradual movement, breathing exercises, and a staged return to food are commonly discussed. Tell the team about increasing abdominal pain, vomiting, fever, wound changes, or difficulty passing stool or gas.

Ask whether the bowel ends were joined, whether a stoma is present, what pathology showed, and which medicines should continue at home. A follow-up plan may include wound review, stoma support, and investigation of the underlying cause. Colonoscopy can be relevant later in selected situations; see [Colonoscopy](/treatments/cancer-screening/colonoscopy/).

When to seek urgent care

Seek emergency care for severe or rapidly worsening abdominal pain, a swollen or rigid abdomen, repeated vomiting, fainting, confusion, bloody stool, or inability to pass stool or gas. After discharge, urgent review is also needed for fever, chills, pus or spreading redness at the wound, heavy bleeding, or a stoma that becomes dark, very swollen, or stops working with worsening pain. These signs need assessment; they do not identify one cause by themselves.

Questions for your doctor

What caused the reduced blood flow, and was all non-viable bowel removed? Were the bowel ends joined or was a stoma required? What did the tissue examination show? Which symptoms should prompt immediate return, and when should wound, bowel, or stoma follow-up happen? If cancer is being considered, ask whether [Bowel Cancer](/conditions/colorectal-cancer/) or [Bowel Resection](/treatments/surgical-oncology/colectomy/) information applies to your situation.

Questions people ask

Is ischemic bowel resection always planned?

No. It may be urgent when examination or imaging suggests that bowel tissue is losing its blood supply or has been injured.

Will I always need a stoma?

No. The decision depends on tissue health, contamination, the operation findings, and whether a safe join can heal.
