Is rotator cuff repair needed before post-operative rehabilitation?

Short answer
Rotator cuff repair is not needed before post-operative rehabilitation; rehabilitation follows surgery when repair has been chosen and prescribed.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Sequence after surgery
- Protect the repair, restore movement, then build strength under guidance
- Key distinction
- Non-operative physiotherapy is not the same as post-operative rehabilitation
How the two fit together
Post-operative rehabilitation means recovery care after an operation. If a surgeon repairs a torn rotator cuff, rehabilitation begins after the procedure and is adjusted to protect the tendon while it heals. It is not a step that must happen before the repair.
A rotator cuff tear does not automatically require surgery. Depending on symptoms, function, tear features and previous treatment, a clinician may recommend activity changes, physiotherapy or observation. Rehabilitation without an operation is a different pathway and should not be treated as a repair protocol.
Making a safe recovery plan
Confirm whether your plan is for surgery followed by rehabilitation or non-operative physiotherapy. After repair, follow the surgeon’s restrictions before starting shoulder exercises; the first stage may protect the tendon and maintain movement in the hand, wrist and elbow.
Keep every surgical and therapy appointment, use the sling as directed and record pain, sleep, hand function and changes in movement. Do not advance resistance or overhead activity without the rehabilitation team’s approval.
When to seek urgent help
Seek prompt clinical advice for increasing wound redness, drainage, fever, sudden loss of movement, new weakness or numbness, or pain that is escalating rather than settling. After a repair, report a fall or sudden force through the arm even if the incision appears normal.
Questions for your doctor
Ask why repair is or is not recommended, what movements are allowed before therapy starts, when formal rehabilitation begins, how progress will be measured, and which symptoms mean the plan needs review.

Questions people ask
Can physiotherapy be used without rotator cuff repair?
Yes. Some shoulder problems are managed without surgery, using a plan matched to symptoms, examination findings and functional goals.
When does rehabilitation begin after repair?
The team usually gives early protection and permitted movements soon after surgery, while the timing and intensity of formal exercises depend on the repair.
