Short answer

Respiratory syncytial virus infection may be mild in infants, but breathing difficulty, dehydration, or unusual sleepiness requires prompt medical assessment.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Possible symptoms
Runny nose, cough, reduced feeding, or fever
Monitor
Breathing effort, colour, alertness, and wet nappies
Urgent signs
Breathing pauses, blue lips, severe effort, or difficult waking

How RSV can affect infants

RSV commonly begins with a runny nose, cough, reduced appetite, or fever. In some infants it irritates the small airways and causes bronchiolitis, with faster breathing, wheezing, or increased effort. Very young babies and infants born early, or those with heart, lung, or immune conditions, can become unwell quickly and need closer observation.

A baby may show illness through feeding changes rather than describing symptoms. Watch breathing, alertness, colour, wet nappies, and the ability to take feeds. Antibiotics do not treat a viral infection unless a clinician identifies another infection.

Supporting your baby

Offer smaller, more frequent feeds if congestion makes feeding difficult, and clear the nose gently with saline drops if advised. Keep the baby away from smoke and from people with respiratory symptoms, and wash hands before touching the baby. Do not give cough medicines or fever medicines without checking the product and dose with a clinician. Continue observing the baby even if symptoms initially seem mild.

When to seek care

Seek urgent help for ribs pulling in with breaths, grunting, pauses in breathing, blue or grey lips, severe wheezing, or a baby who is difficult to wake. Contact a clinician promptly for markedly reduced feeding, fewer wet nappies, worsening cough, persistent fever, or unusual irritability. Any breathing concern in a very young infant deserves a low threshold for assessment.

Questions for your doctor

How hard is my baby working to breathe? Is the feeding amount adequate? How should we monitor wet nappies and alertness? Which symptoms mean emergency care? Does my baby's age or medical history require closer follow-up?
