Why the bump hurts
During growth, the tendon below the kneecap repeatedly pulls on a developing area at the top of the shin. Running, jumping, kneeling, and deep knee bending can irritate that attachment. Pain and tenderness are usually felt at the front of the knee, sometimes with a firm bump. Symptoms may flare during a growth spurt or a busy sports schedule and then settle when loading is reduced.
Managing sport and school
Reduce or pause activities that clearly increase pain, while keeping comfortable daily movement. Ice wrapped in cloth may ease soreness after activity. A physiotherapist can teach thigh, hip, and calf exercises and help plan a gradual return to running or jumping. Supportive footwear and avoiding prolonged kneeling may help. Do not push through limping or use pain medicine to hide symptoms without guidance from a parent, clinician, or pharmacist.
When the knee needs review
Arrange medical assessment if pain follows a significant injury, the knee locks or gives way, swelling is substantial, or the child cannot bear weight. Prompt review is also appropriate for fever, redness, warmth, night pain, pain at rest, or symptoms that keep worsening despite reduced activity. Sudden severe pain should be assessed rather than attributed automatically to Osgood-Schlatter disease.
