Is loss of smell always COVID?

A blocked or inflamed nose can stop smells reaching the smell receptors. Common cold, allergic rhinitis, and chronic sinusitis may do this. COVID-19 can affect smell even when nasal blockage is not prominent, and smell changes can continue after the infection as part of Long COVID. The timing, accompanying symptoms, and local infection guidance help determine the next step.

Note when the change began and whether congestion, sneezing, facial pressure, fever, cough, or taste changes are present. Consider a COVID test according to current local advice, particularly after a recent exposure or new respiratory symptoms. Avoid inserting objects into the nose. Keep smoke and strong irritants away, and arrange review if the change persists or keeps returning.

Seek urgent care for loss of smell after a significant head injury, or with severe headache, new weakness, confusion, or breathing difficulty. Arrange a medical assessment for persistent one-sided blockage, recurrent nosebleeds, severe facial pain, or smell loss that does not improve.

Ask whether your nasal symptoms suggest allergy, sinus disease, or infection; whether testing is useful; and whether a persistent change needs an ear, nose, and throat assessment.
