Is hip fracture surgery needed before post-operative rehabilitation?

A hip fracture can make standing or moving unsafe because the bone and joint are no longer supporting the leg normally. Surgery may realign and secure the fracture, allowing the rehabilitation team to work within the surgeon's weight-bearing instructions. Rehabilitation can begin around the operation, sometimes with breathing, circulation and bed-mobility exercises before walking practice is appropriate.

Not every person follows the same pathway. The fracture's location and stability, medical conditions, medicines, skin condition and ability to take part in therapy can influence timing. In selected situations, a non-surgical plan may be considered, but this requires close clinical supervision.

Ask the treating team what has been injured, whether surgery is advised, and which movements or amount of weight are allowed afterward. Arrange the practical parts of recovery early: a safe route to the bathroom, suitable walking support, help with meals, and a plan for exercises. Follow the prescribed precautions rather than testing the hip by standing or walking alone.

After discharge, rehabilitation may address transfers, balance, leg strength, walking confidence and everyday tasks. Keep a written record of restrictions and appointments so that family members and therapists give consistent support.

Seek urgent medical help after a hip fracture or operation for worsening breathlessness, chest pain, fainting, a new inability to move the leg, severe increasing pain, a visibly changed leg position, or a foot that becomes cold, pale or numb. Contact the surgical team promptly for fever, spreading wound redness, drainage, wound opening, or calf swelling and tenderness.
