Is ACL reconstruction done with an allograft tendon?

Short answer
ACL reconstruction can be performed with an allograft tendon, but graft choice is individual and is not suitable for every patient.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Graft types
- ACL reconstruction may use donor tissue or the patient’s own tendon.
- Choice factors
- Sport, knee findings, prior surgery, and available tissue influence the discussion.
What an allograft means
An allograft is donor tendon tissue prepared for use as a ligament graft. During ACL reconstruction, the surgeon places the graft along the planned ligament path and secures it in the bone tunnels. Other reconstruction options use the patient’s own tendon, called an autograft. The decision considers sporting goals, age, previous operations, available tissue, knee stability, and the surgeon’s assessment. An allograft may avoid taking a tendon from another part of the same leg, but it has its own healing considerations. Read about [Allograft Tendon](/materials/allograft-tendon/) and [ACL Reconstruction](/treatments/sports-and-arthroscopic-surgery/acl-reconstruction/).
Preparing for the decision
Discuss the graft options before surgery and explain the activities you hope to resume. Ask how the graft will be fixed, what rehabilitation restrictions apply, and how the plan changes if meniscus or cartilage treatment is needed at the same time. Pre-operative physiotherapy may help restore knee motion, reduce swelling, and improve muscle control when appropriate. After reconstruction, follow the staged programme even if the knee feels better early; graft healing and neuromuscular recovery continue beyond the first phase.
When to seek care
Before surgery, obtain assessment for a locked knee, repeated giving-way episodes, or substantial swelling after an injury. After surgery, contact the care team for worsening pain, increasing redness or drainage, fever, new calf swelling, or sudden breathlessness. A new instability episode or inability to follow the rehabilitation exercises also deserves review.
Questions for your surgeon
Ask why an allograft or autograft is being recommended for you, what donor-tissue processing involves, how the choice fits your sport, and which milestones will determine return to running and pivoting activities.

Questions people ask
Is an allograft the same as a knee implant?
No. An allograft is prepared donor tendon tissue used to reconstruct the ligament; it is not a metal or plastic joint replacement.
Does an allograft remove the need for rehabilitation?
No. The knee still needs staged motion, strength, balance, and activity retraining after reconstruction.
