Irregular periods and pregnancy signs: what to know

A late bleed may follow delayed ovulation rather than pregnancy. Early pregnancy can involve nausea, tiredness, breast tenderness, increased urination, or light spotting, but these symptoms can overlap with premenstrual changes, stress, and hormone conditions. Perimenopause, polycystic ovary syndrome, anovulation, and premature ovarian insufficiency can also change bleeding patterns. Symptoms alone cannot confirm pregnancy.

If pregnancy is possible, take a home pregnancy test according to its instructions. If the result is negative but the expected timing is unclear, repeat testing later or ask a clinician when to test. Keep a record of bleeding, unprotected sex, contraception, symptoms, and test results. Do not start or stop medicines without advice. A clinician can assess irregular periods and discuss contraception, pregnancy planning, or treatment for a contributing condition; options may include the combined pill or metformin for PCOS when suitable.

Seek urgent medical help for severe or one-sided lower abdominal pain, shoulder-tip pain, faintness, or heavy bleeding when pregnancy is possible. Arrange a review for repeated negative tests with no period, ongoing irregularity, or symptoms that are new or worsening.
