Irregular periods after stopping the Depo shot

Depot medroxyprogesterone suppresses ovulation and can make bleeding very light, irregular, or absent during use. After the last injection wears off, the body may not restart ovulation in a predictable rhythm immediately. Spotting, skipped bleeds, or changing flow can therefore occur. Pregnancy is possible before regular periods return, so a missed bleed is not a reliable way to judge fertility. Other explanations include stress, thyroid disease, anovulation, polycystic ovary syndrome, perimenopause, or premature ovarian insufficiency. The previous injection date, other medicines, and your earlier cycle pattern help a clinician interpret the change.

Write down injection dates, bleeding days, flow, cramps, breast symptoms, and any unprotected sex. Use contraception promptly if you do not want pregnancy; ask about options such as the combined pill, taking account of your medical history. Take a pregnancy test when pregnancy is possible, particularly with nausea, breast tenderness, or a missed expected bleed. Book a review if irregular bleeding continues, becomes troublesome, or is accompanied by pelvic pain, unusual discharge, headaches, vision changes, or symptoms of hormone imbalance. The visit can clarify whether observation, pregnancy testing, examination, or blood tests are appropriate.

Get urgent help for bleeding that rapidly soaks pads, faintness, severe abdominal or pelvic pain, fever, or a positive pregnancy test with pain or bleeding. Seek prompt assessment for chest pain, sudden breathlessness, or a painful swollen leg if you have started an oestrogen-containing contraceptive. Recurrent bleeding after sex or bleeding that interferes with daily life should be assessed even when pain is absent.
