Irregular periods after sex: what to know

Sex can be followed by light spotting from friction or cervical sensitivity, especially if there was dryness, but this is different from a whole cycle becoming irregular. A missed or late period makes pregnancy worth checking if pregnancy is possible. Emergency contraception and starting or changing hormonal contraception can also shift bleeding. Stress, illness, weight changes, thyroid problems, and disrupted ovulation may contribute. Anovulation means an egg was not released during a cycle and can produce unpredictable bleeding. Ongoing patterns may be associated with polycystic ovary syndrome, perimenopause, or premature ovarian insufficiency.

Record the first day, duration, and flow of each bleed, along with spotting after sex, pelvic pain, discharge, contraception, and pregnancy risk. Take a home pregnancy test after a missed period or at the time advised on the test instructions; repeat it if the result is negative but bleeding does not arrive. Avoid douching and use lubrication if dryness may be causing contact spotting. Arrange a clinician visit when irregularity persists, recurs, or comes with acne, excess facial hair, hot flushes, or nipple discharge. A clinician may review medicines, pregnancy risk, infection symptoms, and ovulation before deciding whether examination or tests are needed.

Seek urgent care for heavy bleeding, faintness, severe or one-sided pelvic pain, shoulder-tip pain, fever, or a positive pregnancy test with pain or bleeding. These symptoms need prompt assessment because pregnancy complications and infection require timely attention. Bleeding after sex that repeatedly occurs, bleeding between periods, or new pain during sex also deserves a scheduled medical review.
