Irregular periods after cesarean delivery

After delivery, the reproductive hormones that control ovulation change gradually. Breastfeeding can postpone ovulation and make returning periods unpredictable. When cycles resume, the amount and timing of bleeding may differ from the pattern before pregnancy.

The operation can affect recovery needs, but a cesarean does not automatically explain repeated irregular cycles. Ovulation may return before a period, making pregnancy possible. Anovulation, thyroid disorders, polycystic ovary syndrome, medication changes, and stress are other possible contributors. The bleeding pattern alone cannot identify its cause.

Keep a simple record of bleeding, pain, clots, breastfeeding, contraception, and medicines. Use a pregnancy test when pregnancy is possible. A routine review is sensible if cycles remain irregular, bleeding follows sex, pain persists, or the change is affecting sleep, work, or caring for your baby.

At an appointment, describe the delivery, postpartum bleeding, feeding pattern, and whether the scar is painful or healed. The clinician may consider a pregnancy test, blood tests, or pelvic imaging based on your symptoms. Treatment is chosen according to the cause, future pregnancy plans, and whether you need contraception.

Get urgent help for heavy bleeding with weakness or faintness, severe pelvic or abdominal pain, fever, or a positive pregnancy test with pain or bleeding. Seek prompt advice for foul-smelling discharge, worsening scar pain, bleeding between periods, or bleeding after sex.

Ask whether your feeding pattern, contraception, or medicines could be involved; whether pregnancy, thyroid problems, or anovulation should be checked; and which symptoms require urgent review. You can also ask how a future pregnancy plan changes treatment choices.
