Inverse Psoriatic Arthritis: What to Know

Inverse psoriasis commonly affects moist folds such as the groin, armpits or area beneath the breasts, where plaques may look smooth and bright rather than thick and scaly. Psoriatic arthritis is a separate inflammatory condition involving joints, tendons or areas where tendons attach to bone. A person may have both conditions, but a fold rash alone does not establish arthritis.

When psoriatic arthritis is present, joint pain, joint swelling and morning joint stiffness can occur. Symptoms may affect one or several areas and can change over time. A clinician considers the pattern of symptoms, skin and nail findings, examination and sometimes imaging or blood tests.

Record which joints hurt or swell, how long morning stiffness lasts, and whether movement improves it. Mention any fold rash, scalp or nail changes, eye symptoms and family history of psoriasis. Arrange an assessment with a clinician familiar with inflammatory joint disease rather than assuming every ache is arthritis. Treatment may include anti-inflammatory medication, skin-directed care and physiotherapy, selected according to symptoms and medical history. Gentle movement within comfort can help preserve function while assessment is arranged.

Arrange timely medical review for persistent joint swelling, repeated episodes of inflammatory pain, or stiffness that limits morning activities. Seek urgent care for a painful red eye with light sensitivity or vision change, a suddenly very swollen joint with fever, or rapidly worsening weakness. These symptoms need assessment because they may signal problems beyond a skin-fold rash.
