Intertrigo Between the Buttocks: Treatment and Self-Care

A clinician may recommend a topical antifungal when yeast is contributing, while other rashes may need a different approach. Keeping the cleft clean and thoroughly dry supports healing, but avoid scrubbing or applying several medicated products at once.

Intertrigo is inflammation in a skin fold. Between the buttocks, close contact, sweating, rubbing and limited airflow can leave the skin red, sore, cracked or itchy. Moisture can soften the outer skin and make irritation easier to sustain. The appearance can overlap with eczema, contact dermatitis, psoriasis, hemorrhoidal irritation or other causes of a rash. A clinician may inspect the edges, check for drainage or open skin, and take a sample if the diagnosis is unclear or treatment has not helped.

Rinse gently with lukewarm water or a mild, fragrance-free cleanser, then pat dry rather than rubbing. Change out of damp underwear or clothing promptly and choose breathable, non-tight fabrics. After bathing or sweating, make sure the fold is dry before dressing.

A thin protective barrier may reduce contact with moisture and stool, provided the product does not sting or worsen the rash. Do not use steroid cream, antiseptic, talc or antifungal medicine on broken skin without checking that it is suitable; some products can mask infection or irritate the area. If a clinician confirms fungal involvement, follow the prescribed course of Topical Antifungal Treatment and do not stop simply because itching improves.

Avoid scratching, vigorous wiping and fragranced wipes. If sweating repeatedly triggers the problem, ask whether a moisture-control plan or Prescription Antiperspirant is appropriate for the surrounding skin; products intended for other areas may be too irritating for the cleft.

Arrange a medical assessment if the rash is spreading, repeatedly returning, very painful, cracked, oozing, foul-smelling or not improving with gentle drying and friction reduction. Seek prompt care for fever, rapidly increasing redness or warmth, marked swelling, pus, severe pain, black or grey skin, or feeling faint or unwell. These changes can indicate a deeper or more extensive infection.

Also seek advice sooner if you have diabetes, reduced immunity, poor circulation, or difficulty keeping the area clean and dry. A clinician can distinguish intertrigo from another Skin Rash and select treatment that fits the cause.
