What it means

A bursa is a thin, fluid-filled cushion that helps tissues glide. In the interspinous region, irritation can develop where the bony projections at the back of the spine come close together. Pain may be felt in the midline rather than across the whole back, and bending, arching, prolonged standing or direct pressure may aggravate it. A clinician may examine your spine, movement and tenderness; imaging can be considered when another spinal problem needs to be excluded. What you can do

Reduce activities that clearly trigger the pain for a short period, while keeping gentle movement within your comfort. A physiotherapist can help adjust posture, lifting technique and exercises so the spine is not repeatedly compressed. Your clinician may discuss an Anti-Inflammatory Medication when appropriate for your health history. If symptoms remain focused and persistent, a Cortisone Injection may be considered; Joint Aspiration is a separate procedure used when fluid needs sampling or removal, and is not automatically needed for this condition.

When to seek care

Arrange an assessment if back pain keeps returning, limits sleep or movement, or does not settle after reducing aggravating activity. Seek urgent medical help for new leg weakness, loss of bladder or bowel control, numbness around the saddle area, fever with severe back pain, or pain after a significant injury. These features can point to a problem beyond a local bursa and need prompt evaluation.

No. It describes irritation of a specific bursa, while back pain can arise from muscles, joints, discs, nerves or bone.
