Internal bleeding during pregnancy in the first trimester

Short answer

Internal bleeding during the first trimester cannot be diagnosed from symptoms alone and needs prompt medical assessment, especially with pain, dizziness, or fainting.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Any bleeding in the first trimester should receive clinical advice.

One-sided pain, fainting, dizziness, or shoulder-tip pain.

What the bleeding may mean

Blood may come from the cervix, vagina, uterus, or a pregnancy-related problem. Light spotting can occur when the cervix is sensitive, but bleeding can also accompany miscarriage or an ectopic pregnancy, in which a pregnancy develops outside the uterus. Bleeding may be visible, or blood can collect internally and cause increasing pain or pressure without much vaginal loss. An examination and ultrasound help locate the pregnancy and assess the source. Blood tests may also help clinicians follow pregnancy hormone changes and check your blood group.

What to do now

Contact your maternity team, obstetrician, or an urgent care service as soon as you notice bleeding. Note when it began, whether it is spotting or flowing, its colour, and whether clots or tissue are present. Tell the clinician about one-sided pain, shoulder-tip pain, fever, recent sex, previous pregnancy complications, and your blood group if known. Use a pad to observe the amount rather than a tampon, and avoid driving if you feel weak or light-headed. An [Ectopic Pregnancy](/conditions/ectopic-pregnancy/) can become an emergency and must be assessed promptly.

When to seek emergency care

Call emergency services or go to an emergency department for severe or one-sided abdominal pain, shoulder-tip pain, fainting, marked dizziness, confusion, pale or clammy skin, or bleeding that is rapidly increasing. These signs can indicate internal blood loss or another urgent pregnancy complication. Do not wait for a routine appointment if you feel suddenly unwell. Even mild bleeding deserves same-day clinical advice during early pregnancy, because the amount of blood seen does not reliably show what is happening internally.

Questions for your doctor

What tests will locate the pregnancy and identify the bleeding source? When should the ultrasound or blood tests be repeated? Which symptoms mean I should return immediately? Do I need treatment or monitoring for my blood group?

Questions people ask

Can internal bleeding happen without heavy vaginal bleeding?

Yes. Blood can collect inside the abdomen, so pain, weakness, dizziness, or fainting may be more revealing than the amount seen externally.

Does first-trimester bleeding always mean pregnancy loss?

No. Several causes are possible, but only an assessment can clarify the situation and identify urgent complications.
