Intermittent cloudy urine: common explanations and next steps

Urine can look cloudy when it is concentrated, especially after not drinking enough. A urinary tract infection may cause cloudiness with burning, urgency, frequent trips, lower abdominal discomfort, or an unusual smell. Kidney infection can cause fever, chills, pain in the side or back, nausea, or feeling markedly unwell. Cloudiness can also reflect crystals, mucus, semen, vaginal discharge, or contamination of a sample. The appearance alone cannot distinguish these causes.

Notice whether the change occurs after waking, exercise, sex, particular foods, or reduced fluid intake. Drink regularly unless a clinician has advised fluid restriction, and collect a clean midstream urine sample if testing is requested. Do not use leftover antibiotics. If cloudiness repeats or comes with burning, urgency, pain, or smell, arrange a medical review; a urine test can help guide treatment.

Seek urgent care for cloudy urine with fever, shaking chills, flank pain, vomiting, confusion, faintness, or visible blood. Pregnancy, childhood, older age, kidney disease, or a weakened immune system can lower the threshold for prompt assessment.
