What intercritical gout means

A flare can end before the underlying urate burden has been addressed. During this interval, the joint may feel normal, yet crystal deposits can continue to irritate the joint lining and make another flare possible. Gout commonly affects the big toe, but ankle, knee and other joints can also be involved. A lack of pain does not by itself show that gout activity has ended.

Kidney disease and obesity can be relevant to a gout assessment because they may affect urate handling or overall joint load. Your clinician may review symptoms, medicines, kidney function and urate testing when deciding whether long-term treatment is appropriate.

What you can do between flares

Arrange a review after a flare has settled, especially if attacks recur, involve a new joint or interfere with walking. Take prescribed urate-lowering treatment consistently; do not start, stop or change it without advice. Keep a note of the joint involved, possible triggers, medicines and how quickly symptoms developed. Regular hydration, balanced meals and gradual weight management may support the wider plan, but these steps do not replace prescribed care.

When to seek care

Seek urgent medical advice for a hot, very swollen joint with fever, feeling faint or being generally unwell, because infection can resemble a gout flare. Get prompt help if pain begins after an injury, you cannot bear weight, or swelling is rapidly worsening. Contact your clinician if attacks keep returning, treatment is difficult to tolerate, or symptoms do not settle as expected.
