Intercourse during ovulation pain: what to know

Ovulation pain, also called mittelschmerz, can cause one-sided lower abdominal or pelvic discomfort around the middle of a cycle. Deep penetration may increase pressure in the area, while orgasm can briefly intensify cramping for some people. The pain should settle rather than steadily worsen.

Ovulation pain is a symptom, not a diagnosis for every episode of pelvic pain. Endometriosis or an ovarian cyst can also cause pain during sex, and their patterns may not follow ovulation.

Choose rest or non-penetrative intimacy until movement feels comfortable. If you do have sex, go slowly, use positions with shallow penetration and stop when pain rises. A warm bath or heat pack may ease mild cramping. Record the cycle day, side, duration and triggers so a clinician can see a pattern.

If pain returns often, affects sex or does not settle, arrange a gynaecology assessment. Do not rely on pain timing alone to rule out another pelvic condition.

Seek urgent medical help for sudden severe pain, faintness, fever, vomiting, heavy bleeding or pain with a possible pregnancy. Arrange prompt care for new pain during sex, worsening episodes, unusual discharge or pain that continues beyond the expected mid-cycle window.
