Pain that recurs on one side around the same cycle stage and settles without other symptoms can fit ovulation pain. Pain from endometriosis may follow a broader pattern, including painful periods, pain during sex, or bowel and bladder discomfort. Ovarian cysts can cause pressure or one-sided pain and may sometimes change suddenly. The timing of pain is useful information, but it does not establish a diagnosis.

Record the date, side, severity, duration, bleeding, bowel or urinary symptoms, and any chance of pregnancy. Rest, gentle heat, fluids, and an appropriate over-the-counter pain medicine may help if a clinician has said it is safe for you. Do not keep treating severe recurring pain at home without review. Assessment may include an examination, pregnancy testing, or pelvic imaging based on your symptoms.

Seek urgent medical help for sudden severe pain, fainting, dizziness, shoulder-tip pain, heavy bleeding, fever, vomiting, a rigid abdomen, or pain with a positive pregnancy test. Arrange an appointment when pain is new, worsening, lasts longer than expected, interrupts normal activities, or occurs with painful periods, unusual discharge, or pain during sex.
