What should you know about instrumental vaginal delivery?

Before the procedure, the bladder is emptied and pain relief is offered. The clinician places the chosen instrument around the baby's head and uses it during contractions while you push. Forceps are shaped instruments; a vacuum cup attaches gently to the baby's scalp. If assistance does not work or safety changes, a caesarean birth may be recommended. A newborn may have temporary scalp marking or facial pressure, while you may have soreness, swelling, or a tear.

Ask what is prompting the recommendation, which instrument is planned, and what you may feel. After birth, follow instructions for wound care, pelvic-floor exercises, pain relief, and keeping the area clean. Rest when possible and tell the maternity team if urination, bowel movements, sitting, or movement becomes difficult. A lactation consultation can help if discomfort or the baby's early examination makes feeding harder.

Contact your maternity service urgently for heavy or increasing bleeding, fever, worsening pain, foul-smelling discharge, inability to pass urine, or a wound that is opening. Seek immediate help for severe headache, breathlessness, chest pain, fainting, or sudden one-sided leg swelling. For the baby, promptly report poor feeding, unusual sleepiness, breathing trouble, or a swelling on the scalp that is enlarging.
