What may be behind it

Pain around the big-toe side may occur with bunions or hallux rigidus, while arch discomfort can accompany flat feet or plantar fasciitis. A change in mileage, hills, speed, surface, or shoes can overload the tissues supporting the inside of the foot. Pain that is sharply localised over a bone, especially with swelling or pain during walking, can occur with a stress fracture. Hammer toe can add pressure in the front of the foot.

What to do after the run

Stop running until ordinary walking is comfortable and does not worsen the pain. Avoid testing the foot with another hard session. Wear supportive shoes with room for the toes, and choose gentle activity only when it stays comfortable. A wrapped cold pack may ease a recent flare for short periods.

Review recent training and footwear changes. A clinician can assess alignment, joint motion, tendon loading, strength, and gait. Custom orthotics may help selected mechanical problems, but persistent pinpoint pain needs evaluation before you return to impact.

When to seek care

Arrange an assessment if pain recurs after every run, lasts beyond a short period of reduced impact, or limits walking. Seek urgent care for inability to bear weight, marked swelling, bruising, deformity, spreading redness, fever, new numbness, or severe focal pain after a sudden increase in impact.
